Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment
Smile 4 Less Orthodontics is required by law to maintain the privacy of your protected health information ("PHI"), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
- Treatment. To provide, coordinate, and manage your orthodontic care, including sharing information with your dentist, physician, or other providers involved in your care.
- Payment. To bill and collect payment for services, including submitting claims to your dental insurance and verifying coverage.
- Health care operations. To run our practice: quality review, training, scheduling, and business planning.
- Appointment reminders and treatment information. We may contact you by phone, text, mail, or email to remind you of appointments or tell you about treatment options.
- Family and friends. We may share information with a family member or other person involved in your care or payment, unless you object.
- As required by law, for public health activities, health oversight, judicial and administrative proceedings, law enforcement, and to avert a serious threat to health or safety.
Other uses and disclosures, including most uses for marketing and any sale of PHI, require your written authorization, which you may revoke at any time in writing.
Your rights
- Inspect and copy your health record (a reasonable copying fee may apply).
- Request an amendment if you believe information is incorrect or incomplete.
- Request an accounting of certain disclosures we've made.
- Request restrictions on certain uses and disclosures. We must agree to restrict disclosures to a health plan for services you paid for in full out of pocket.
- Request confidential communications by alternate means or at an alternate location.
- Receive a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Be notified if there is a breach of your unsecured PHI.
Changes to this notice
We reserve the right to change this notice and make the new notice effective for all PHI we maintain. The current notice is posted in each office and on this website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
